群発頭痛症例　NO４６　プレドニンで群発頭痛をこじらせてしまった。

今回はプレドニン(合成副腎皮質ホルモン剤)によって群発頭痛をこじらせてしまい、群発期が終わらなくなってしまった方の症例をご紹介します。
プレドニンは時に神の薬と崇められる程の劇的な効果を発揮しします。
しかし多くの症例をみているうちに、プレドニンは諸刃の剣となってしまうことがわかってきました。
今症例はその典型です。あの激痛から救ってくれる神の薬ですが、実は悪魔の薬になることをしっかりとご理解頂きたいと思います。

20代　女性
約１０年前に群発頭痛を発症。昼夜不規則なお仕事をされていたのが、発症の原因の一因になったと思われる。
前期もプレドニンを使用し、すぐに症状が治まり、またぶり返すようなことはなかった。
プレドニンを使用するのは今期が２期目で、今回も苦しむことなく乗り切れると思っていた。

来院されるまでの経過
来院される半年前から群発期突入。
前期同様、すぐに病院へ行きプレドニンを処方してもらった。
プレドニンの効果はすぐにあり、安心していたが、服用期間が終了してしばらく(１～２週間)でまた発作が始まってしまった。そこでまたプレドニン処方してもらったが、同様に服用期間が終了するとまた数日で発作が始まってしまった。ドクターに渋られたが、その後またすぐに３回目のプレドニン服用をスタート。またピタッと発作が治まった。
初回来院時はプレドニン残り３日で、発作が出ていない状態。

症状経過
初回
群発頭痛の症状は右のみに出るが、首肩のコリ感は左右とも強く出ている。

３日後　２回目の施術
昨日でプレドニン終了。左右首肩のコリ感は相変わらず強い。発作なし。

５日後　３回目の施術
頭痛なし。首肩のコリ感は残存。今期の今までの流れを考えると、そろそろ発作が起きるのではないかと推測。

６日後　４回目の施術
発作なしだが、夜中に右目の奥に違和感を感じて起きてしまう。違和感で止まるこの状態は症状経過が順調であることを表しています。さらに左首のコリ感は消失。右首も軽くなってきた。

７日後　５回目の施術
発作なし。右首のコリ感は残存するものの、この時点で発作が起きないという事は群発期が終息に向かっていると判断し集中施術は終了する。次回は１カ月後の予防施術。

考察
プレドニンやイミグラン点鼻・皮下注射で群発頭痛をこじらせてしまうと、施術の効果に１番大きな悪影響となります。風邪やその他の病気もこじらせると治りにくなりますね。群発頭痛も同じです。
正直申し上げまして、今回も改善までに時間が掛かってしまう事を予想し、クライアント様にもその旨をお伝えしていました。しかし１度もぶり返すことなく群発期を終了できたことは、ラッキーとしか言いようがありません。
当たり前ですが施術の効果に個人差があります。
今症例のように全て順調に良くなってしまえばいいのですが、そういうわけにいかないことも間違いなくあります。
ただ粘り強く、諦めず続けていけば、ほとんどの場合改善していきます。

However, while looking at many cases, it turned out that predonin becomes a double-edged sword.
The case is now typical. It is God’s medicine that will save you from that severe pain, but I would like you to really understand that it will be a devil’s medicine.

Twenties women
A cluster headache occurs about 10 years ago. The irregular work during the day and night seems to have contributed to the cause of the onset.
In the previous period, he used predonin, and his symptoms went away quickly, and he never relapsed.
It was my second term to use predonin, and I thought I could survive without suffering this time.

The course until the hospital visit </ strong>
The swarming phase rushed from half a year before coming to the hospital.
As in the previous term, I immediately went to the hospital and prescribed predonin.
The effect of predonin was immediate, and I was relieved, but the seizure started again after a while (1 to 2 weeks) after the dosing period. So I got a prescription for predonin again, but when the dosing period ended, the seizure started again in a few days. She was reluctant by the doctor, but soon after that she started taking the third prednine dose. His seizures got better again.
At the time of the first visit, there is no seizure in 3 days of predonin remaining.

Symptoms progress </ strong>
First time
The symptoms of cluster headache appear only on the right, but the stiffness of the neck and shoulder is strong on both sides.

3 days later the second treatment
End of predonin yesterday. The feeling of stiffness on the left and right neck shoulders is still strong. No seizures.

5 days later the third treatment
No headache A feeling of stiffness of the neck and shoulders remains. Considering the current flow of this term, we speculate that a seizure will occur.

Six days later the fourth treatment
There is no seizure, but I feel discomfort in the back of my right eye in the middle of the night and get up. This condition of stopping with a sense of discomfort indicates that the symptom has progressed smoothly. Furthermore, the feeling of stiffness on the left neck disappears. The right neck has also become lighter.

Seven days later, the fifth treatment
No seizures. Although the feeling of stiffness in the right neck remains, the fact that the seizure does not occur at this time is judged that the swarm period is ending, and the intensive treatment ends. The next month is a preventive treatment one month later.

Discussion </ strong>
If the group headache is caused by predonin or imigran intranasal and subcutaneous injections, the effect of the treatment will be the biggest adverse effect. You can get rid of colds and other illnesses as well. Cluster headaches are the same.
To be honest, I also predicted that it will take time to improve this time, and told that to the client. However, being able to end the swarming phase without having to go back again, I can say nothing but lucky.
It is natural, but there are individual differences in the effect of the treatment.
It is good if all things go well as in the case, but there is no doubt that it can not be done that way.
However, if you keep going and keep giving up, you will improve in most cases.
By all means, I believe in improvement and I would like you to continue the treatment.
