群発頭痛症例　NO２４　いつもなら１カ月で終わるはずが…

４０代　男性　東京都　豊島区

・１０歳位のときに群発頭痛を発症。診断を受けたのは８年前。２年に１回の群発期で、通常１カ月で終わる。今回は発作の回数が非常に多く、４～５回で１～２時間続く。１カ月経ったにも関わらず、ずっとピークの状態が続き地獄のような状態。出口の見えないトンネルに疲れ切ったご様子。イミグラン皮下注射とイミグランのジェネリックであるスマトリプタンを使用。ひどいときは皮下注射、軽い時はスマトリプタン。

［経過］

・初診時まさに発作が起き始めた状態。レベル８位でスマトリプタンを飲んだが抑えることが出来ず、これからMAXが来てしまいそうな状況。説明よりも先に施術。施術によりうまく群発頭痛発作が回避でき、驚かれていました。その場で発作が抑えられるパターンはほぼ間違いなく、その後の経過が順調です。

・翌日に続けて２回目の施術。群発頭痛発作回数、痛みレベル減。イミグラン皮下注射使用なし。

・３日後に３回目の施術。１日発作が２回起きてしまうが、薬の効きがとても良く、痛みも弱い。イミグラン皮下注射は１回使用。

・６日後に４回目の施術。モヤモヤ来るが発作は起こらなくなったため様子見とする。

今回は痛みはピークでしたが、群発期は終盤での施術開始となりました。まだ周期的に改善するような感じではないので、次回の群発期に初期から施術を開始し、また順調な経過を辿ることを願います。こういったケースではより確実に効果を上げるために定期的な調整をおすすめします。

Cluster headache treatment　NO２４

· Cluster headache developed at around 10 years old. I got a diagnosis eight years ago. It is in the cluster phase once every two years, usually ending in a month. This time the number of attacks is very large, lasting 1 to 2 hours in 4 to 5 times. Even though it passed one month, the state of the peak has continued all the time and it is like a hell. I was exhausted by the invisible tunnel at the exit. Immigran use subcutaneous injection and imaglan generic sumatriptan. Subcutaneous injection in severe cases, sumatriptan when light.

Treatment course

· At the time of the first visit it is just the beginning of a seizure. I drank Sumatriptan at the 8th place but I could not suppress it, and it seems that MAX will come from now. Treat treatment earlier than explanation. I was surprised that clinical headache attack could be successfully avoided by treatment. There is almost no doubt the pattern that seizures can be suppressed on the spot, and the subsequent course of treatment is smooth.

· Second treatment following the next day. Cluster headache attack frequency, pain level reduction. Immigran hypodermic injection Not used.

· Third treatment after 3 days. Although I have a one-day seizure twice, the effect of the medicine is very good and the pain is weak. Immigran hypodermic injection was used once.

· Fourth treatment after 6 days. Moyamoya comes but seizure does not occur, so I will wait.

Although the pain was a peak this time, treatment started at the end of the cluster period. Since it is not feeling that it will be cured periodically yet, I hope that you will be treated from the beginning in the next cluster phase and follow a steady progress. In these cases, we recommend periodic adjustment to make the effect more reliable.
